My Council Record Checklist

Below is a checklist of information needed to submit your Council Record for verification.

You may submit an entry into your Council Record for verification at any time. All sections (education, employment, registration, and examination) must be verified before a transmittal can be sent to your licensure board.

My Education

  • Institution name
  • Education level
  • Dates of attendance 
  • Transcript sent directly from your institution to CLARB​

My Employment

  • Employment type
  • Weekly hours
  • Employment dates
  • Company name
  • Supervisor name
  • Supervisor email address
  • Supervisor phone number

My Registration

  • Jurisdiction
  • Licensure number
  • Registration dates (if applicable) 

My Examination

  • Applicants with exam results obtained prior to 2000 should email info@clarb.org with their exam history information.
  • Applicants with exam results after 2000, your exam history will automatically display in your Council Record and within a transmittal to your licensure board.
